The native hormone is produced in the hypothalamus and acts on the anterior pituitary. Binding of GHRH to its receptor stimulates synthesis and release of growth hormone into circulation. Because the analogue retains the receptor-binding region of the parent sequence, it engages the same receptor and triggers the same downstream signaling. The result is increased growth hormone secretion from pituitary cells, which in turn influences hepatic production of insulin-like growth factor 1. This axis is the basis for the compound's measured biological effects.
Interest in this peptide developed because native GHRH has a short circulating lifetime. The N-terminal modification slows cleavage by dipeptidyl peptidase IV, an enzyme that removes the first two residues of many peptides and terminates their activity. Slower degradation means a longer window of receptor stimulation per administration. This design logic parallels other modified peptide hormones, where a small chemical change at a vulnerable site yields a more durable molecule without altering the core mechanism of action.
The peptide is synthesized chemically rather than extracted from biological sources. Solid-phase synthesis builds the chain from the C-terminus toward the N-terminus, after which the hexenoyl group is attached. Purity is typically assessed by high-performance liquid chromatography, and identity is confirmed by mass spectrometry. Regulatory review of the finished product focuses on these analytical controls, since small deviations in sequence or modification can change biological activity. Questions about long-term effects on the pituitary axis remain areas of continued investigation.
Tesamorelin is a synthetic peptide analog of growth hormone-releasing hormone, composed of 44 amino acids. It was designed to retain the biological activity of the native hormone while resisting rapid enzymatic degradation. The compound is classified as a growth hormone secretagogue and belongs to the broader family of hypothalamic releasing factors. In research and clinical settings, it is studied for its ability to stimulate pituitary growth hormone release. Its structure includes a modification at the N-terminus that contributes to an extended half-life relative to native growth hormone-releasing hormone.
Tesamorelin binds to growth hormone-releasing hormone receptors on the surface of pituitary somatotroph cells. This binding activates adenylate cyclase, raising intracellular cyclic AMP levels and triggering the release of growth hormone into circulation. The elevated growth hormone then stimulates hepatic production of insulin-like growth factor 1. Because the effect is mediated through the endogenous axis, secretion remains subject to feedback regulation. This distinguishes it from direct growth hormone administration, which bypasses pituitary control entirely.
Clinical investigation has focused on HIV-associated lipodystrophy, a condition in which antiretroviral therapy contributes to abnormal fat distribution. Excess visceral adipose tissue accumulates in the abdomen while peripheral fat may be lost. Tesamorelin was evaluated for reducing this visceral fat depot, with trials measuring changes in abdominal fat by imaging rather than by body weight alone. The rationale rests on the known lipolytic effects of growth hormone. Effects on visceral fat are documented, while long-term outcomes regarding cardiovascular risk remain less clearly established.

Following the ban on Sufi orders in the Republic of Turkey, the Bektashi community's headquarters was moved from Hacıbektaş in central Anatolia, to Tirana, Albania. In Albania, the Bektashi community declared its separation from the Sunni community and they have since perceived as a distinct Islamic sect rather than a branch of Sunni Islam. Among the most famous followers of Bektashi in Balkans during the 19th century were Ali Pasha of Yanina and Naim Frashëri.

=== Skin === One form the rashes take is called "heliotrope" (a purplish color) or lilac, but may also be red. It can occur around the eyes along with swelling, but also occurs on the upper chest or back what is called the "shawl" (around the neck) or "V-sign" above the breasts and may also occur on the face, upper arms, thighs, or hands. Another form the rash takes is called Gottron's sign, which is red or violet, sometimes scaly, slightly raised papules that erupt on any of the finger joints (the metacarpophalangeal joints or the interphalangeal joints). Gottron's papules may also be found over other bony prominences including the elbows, knees, or feet. All these rashes are made worse by exposure to sunlight, and are often very itchy, painful, and may bleed. If a person exhibits only skin findings characteristic of DM, without weakness or abnormal muscle enzymes, may be classified as having amyopathic or clinically amyopathic dermatomyositis (ADM), formerly known as "dermatomyositis sine myositis".

=== Congenital Disorders of Amino Acid Metabolism === Newborn screening originated with an amino acid disorder, phenylketonuria (PKU), which can be easily treated by dietary modifications, but causes severe Intellectual disability if not identified and treated early. Robert Guthrie introduced the newborn screening test for PKU in the early 1960s. With the knowledge that PKU could be detected before symptoms were evident, and treatment initiated, screening was quickly adopted around the world. Ireland was the first country in the world to introduce a nationwide screening programme in February 1966, Austria started screening the same year and England in 1968. Other congenital disorders of amino acid metabolism tested for on the newborn screening include Tyrosinemia and Maple Syrup Urine Disorder.
== History == The Tetra Brik package was introduced in 1963, after a long process of development and built on the previous Tetra Classic tetrahedron package that laid the foundation for Tetra Pak. Despite the revolutionary character of the new retail system, by the end of the 1950s Ruben Rausing and the Tetra Pak management team realised that the Tetra Pak package portfolio needed to be supplemented by an additional rectangular model to continue to be competitive. Large amounts were spent on development, and in 1963 the first Tetra Brik packaging machine was installed in Motala in central Sweden. Due to its effective use of space and materials and the increased efficiency in distribution and storage that resulted from the rectangular shape, the Tetra Brik soon became Tetra Pak's best seller in Sweden and internationally and paved the way for Tetra Pak's enormous success during the 1970s and onwards. The Tetra Brik package family has since been extended and is currently composed of Tetra Brik and Tetra Brik Aseptic in the shapes Base, Square and Edge, ranging from volumes of 80 to 2000 ml and with a number of different caps. Tetra Brik has been distinguished as one of the 20th century's design icons. It featured in the 2004 Museum of Modern Art New York exhibition Humble Masterpieces and in the 2011 London Science Museum/Vitra Design Museum exhibition Hidden Heroes – The Genius of Everyday Things. It is now part of the permanent collection at the MoMA in New York.
The Republic of Korea Armed Forces issues two types of field rations, Type I and Type II. Type I ration has ready-to-eat foods packed in foil-plastic trilaminate pouches, placed in turn inside a thin cardboard box. Typical contents include: 1 pouch (250 g) precooked white rice with meat and vegetables, plus a separate seasoning packet; 1 pouch (250 g) precooked rice with red beans; 1 packet (100 g) of 6 pork sausages in BBQ; 1 packet (100 g) kimchi; and 1 packet (50 g) cooked black beans. The Type II ration is a smaller, lighter, freeze-dried single-meal ration consisting of several small pouches packed inside a larger gray plastic pouch measuring 225 mm x 200 mm x 90 mm and weighing 278 g. Typical contents include: freeze dried rice (various flavors, usually with meat and vegetables included), a pouch of instant soup, flavored sesame oil, seasoning and spice packets, dried chives and chocolate.

==== 1980s–early 1990s ==== The modern use of the term designer drug was coined in the 1980s to refer to various synthetic opioid drugs, based mostly on the fentanyl molecule (such as α-methylfentanyl). The term gained widespread popularity when MDMA (ecstasy) experienced a popularity boom in the mid-1980s. When the term was coined in the 1980s, a wide range of narcotics were being sold as heroin on the black market. Many were based on fentanyl or meperidine. One, MPPP, was found in some cases to contain an impurity called MPTP, which caused brain damage that could result in a syndrome identical to late stage Parkinson's disease, from only a single dose. Other problems were highly potent fentanyl analogues that caused many accidental overdoses. Because the government was powerless to prosecute people for these drugs until after they had been marketed successfully, laws were passed to give the DEA power to emergency schedule chemicals for a year, with an optional 6-month extension, while gathering evidence to justify permanent scheduling, as well as the analogue laws mentioned previously. Emergency-scheduling power was used for the first time for MDMA. In this case, the DEA scheduled MDMA as a Schedule I drug and retained this classification after review, even though their own judge ruled that MDMA should be classified Schedule III on the basis of its demonstrated uses in medicine. The emergency scheduling power has subsequently been used for a variety of other drugs including 2C-B, AMT, and BZP.
PHGDH is allosterically regulated by its downstream product, L-serine. This feedback inhibition is understandable considering that 3-phosphoglycerate is an intermediate in the glycolytic pathway. Given that PHGDH represents the committed step in the production of serine in the cell, flux through the pathway must be carefully controlled. L-serine binding has been shown to exhibit cooperative behavior. Mutants that decreased this cooperativity also increased in sensitivity to serine's allosteric inhibition, suggesting a separation of the chemical mechanisms that result in allosteric binding cooperativity and active site inhibition. The mechanism of inhibition is Vmax type, indicating that serine affects the reaction rate rather than the binding affinity of the active site. Although L-serine's allosteric effects are usually the focus of regulatory investigation, it has been noted that in some variants of the enzyme, 3-phosphoglycerate dehydrogenase is inhibited at separate positively charged allosteric site by high concentrations of its own substrate.

It shares the 44-residue sequence of human GHRH but carries an added trans-3-hexenoyl group at its N-terminus. That addition does not occur in the natural hormone and serves mainly to resist enzymatic breakdown. The receptor target and signaling pathway remain the same.
It binds the growth hormone-releasing hormone receptor on anterior pituitary cells. Activation of that receptor promotes synthesis and release of growth hormone. The effect propagates through the growth hormone and insulin-like growth factor 1 axis.
Native GHRH is cleared quickly by peptidases, which limits how long it can stimulate its receptor. The added group hinders one of the primary cleavage enzymes. The practical consequence is a longer period of receptor activity per dose.
It is a synthetic analog of growth hormone-releasing hormone, a hypothalamic peptide. It functions as a growth hormone secretagogue acting at pituitary receptors. The classification separates it from direct growth hormone products.
