Development work on the compound, originally designated TH9507, focused on conditions in which reduced growth hormone signaling is thought to contribute to altered body composition. The United States Food and Drug Administration approved it in 2010 for the treatment of excess visceral abdominal fat in adults with human immunodeficiency virus infection and lipodystrophy. Later research examined other populations, including adults with mild cognitive impairment, where a large trial did not meet its primary endpoints. This mixed record illustrates how a single mechanism can produce clear effects in one setting and inconclusive results in another.
Several related peptides act on the same receptor, including sermorelin, a shorter GHRH fragment, and modified analogs such as CJC-1295 and modified GRF(1-29) that are common in research settings rather than approved products. Tesamorelin differs from growth hormone itself in that it acts upstream, prompting the pituitary to release the hormone through physiological signaling rather than supplying it directly. Terminology in the literature distinguishes GHRH analogs, growth hormone secretagogues, and recombinant growth hormone, although popular discussion often blurs these categories together. Precise naming matters when comparing study results.
Tesamorelin is a synthetic peptide of forty-four amino acids whose sequence reproduces human growth hormone-releasing hormone. Its distinguishing feature sits at the amino terminus, where a trans-3-hexenoyl group replaces the free amine. That acylation slows cleavage by dipeptidyl peptidase IV, an enzyme that otherwise removes the first two residues and inactivates the natural hormone quickly. The modified peptide therefore persists longer in circulation while keeping the same receptor target. It is handled as a lyophilized solid and dissolved shortly before use.
Signaling begins at the GHRH receptor, a class B G protein-coupled receptor displayed on somatotroph cells of the anterior pituitary. Receptor occupancy activates Gs proteins, which raise adenylyl cyclase activity and intracellular cyclic AMP, in turn driving protein kinase A dependent pathways. The downstream output is synthesis and pulsatile secretion of growth hormone into the bloodstream. Hepatic tissue and peripheral sites respond by increasing insulin-like growth factor 1 production. Somatostatin and IGF-1 itself supply negative feedback that caps the size and duration of each secretory burst.
Marsha Linehan's biosocial developmental theory posits that BPD arises from the interaction between a child's inherent emotional vulnerability and an invalidating environment—an environment characterized by the neglect, ridicule, dismissal, or discouragement of a child's emotions and needs. Due to it drawing on attachment theory, which centers Western nuclear family norms, the diagnosis of BPD has been critiqued by Indigenous and decolonial scholars for possibly pathologizing Indigenous and collectivist cultural forms of caregiving.

Melanin: It is brown in colour and present in the basal layer of the epidermis. Melanoid: It resembles melanin but is present diffusely throughout the epidermis. Carotene: This pigment is yellow to orange in colour. It is present in the stratum corneum and fat cells of dermis and superficial fascia. Hemoglobin (also spelled haemoglobin): It is found in blood and is not a pigment of the skin but develops a purple colour. Oxyhemoglobin: It is also found in blood and is not a pigment of the skin. It develops a red colour. There is a correlation between the geographic distribution of UV radiation (UVR) and the distribution of indigenous skin pigmentation around the world. Areas that highlight higher amounts of UVR reflect darker-skinned populations, generally located nearer towards the equator. Areas that are far from the tropics and closer to the poles have lower concentration of UVR, which is reflected in lighter-skinned populations. In the same population it has been observed that adult human females are considerably lighter in skin pigmentation than males. Females need more calcium during pregnancy and lactation, and vitamin D, which is synthesized from sunlight, helps in absorbing calcium. For this reason it is thought that females may have evolved to have lighter skin in order to help their bodies absorb more calcium. The Fitzpatrick scale is a numerical classification schema for human skin colour developed in 1975 as a way to classify the typical response of different types of skin to ultraviolet (UV) light:
=== Antithrombotics === Aspirin, an antiplatelet drug, is given as a loading dose to reduce the clot size and reduce further clotting in the affected artery. It is known to decrease mortality associated with acute myocardial infarction by at least 50%. P2Y12 inhibitors such as clopidogrel, prasugrel and ticagrelor are given concurrently, also as a loading dose, with the dose depending on whether further surgical management or fibrinolysis is planned. Prasugrel and ticagrelor are recommended in European and American guidelines, as they are active more quickly and consistently than clopidogrel. P2Y12 inhibitors are recommended in both NSTEMI and STEMI, including in PCI, with evidence also to suggest improved mortality. Heparins, particularly in the unfractionated form, act at several points in the clotting cascade, help to prevent the enlargement of a clot, and are also given in myocardial infarction, owing to evidence suggesting improved mortality rates. In very high-risk scenarios, inhibitors of the platelet glycoprotein αIIbβ3a receptor such as eptifibatide or tirofiban may be used. There is varying evidence on the mortality benefits in NSTEMI. A 2014 review of P2Y12 inhibitors such as clopidogrel found they do not change the risk of death when given to people with a suspected NSTEMI prior to PCI, nor do heparins change the risk of death. They do decrease the risk of having a further myocardial infarction.
== Function == Several lines of evidence suggest that PD-1 and its ligands negatively regulate immune responses. PD-1 knockout mice have been shown to develop lupus-like glomerulonephritis and dilated cardiomyopathy on the C57BL/6 and BALB/c backgrounds, respectively. In vitro, treatment of anti-CD3 stimulated T cells with PD-L1-Ig results in reduced T cell proliferation and IFN-γ secretion. IFN-γ is a key pro-inflammatory cytokine that promotes T cell inflammatory activity. Reduced T cell proliferation was also correlated with attenuated IL-2 secretion and together, these data suggest that PD-1 negatively regulates T cell responses. Experiments using PD-L1 transfected DCs and PD-1 expressing transgenic (Tg) CD4+ and CD8+ T cells suggest that CD8+ T cells are more susceptible to inhibition by PD-L1, although this could be dependent on the strength of TCR signaling. Consistent with a role in negatively regulating CD8+ T cell responses, using an LCMV viral vector model of chronic infection, Rafi Ahmed's group showed that the PD-1-PD-L1 interaction inhibits activation, expansion and acquisition of effector functions of virus specific CD8+ T cells, which can be reversed by blocking the PD-1-PD-L1 interaction. Expression of PD-L1 on tumor cells inhibits anti-tumor activity through engagement of PD-1 on effector T cells. Expression of PD-L1 on tumors is correlated with reduced survival in esophageal, pancreatic and other types of cancers, highlighting this pathway as a target for immunotherapy.

Oral selegiline was significantly more effective than the selegiline patch in terms of depressive symptom improvement (SMD = −1.49, k = 6, n = 282 vs. SMD = −0.27, k = 4, n = 1,026, respectively; p = 0.03). However, this was largely due to older and less methodologically rigorous trials that were at high risk for bias. Oral selegiline studies also often employed much higher doses than usual, for instance 20 to 60 mg/day. The quality of evidence of selegiline for depression was rated as very low overall, very low for oral selegiline, and low to moderate for transdermal selegiline. For comparison, meta-analyses of other antidepressants for depression have found a mean effect size of about 0.3 (a small effect), which is similar to that with transdermal selegiline. In two pivotal regulatory clinical trials of 6 to 8 weeks duration, the selegiline transdermal patch decreased scores on depression rating scales (specifically the 17- and 28-item HDRSTooltip Hamilton Depression Rating Scale) by 9.0 to 10.9 points, whereas placebo decreased scores by 6.5 to 8.6 points, giving placebo-subtracted differences attributable to selegiline of 2.4 to 2.5 points. A 2013 quantitative review of the transdermal selegiline patch for depression, which pooled the results of these two trials, found that the placebo-subtracted number needed to treat (NNT) was 11 in terms of depression response (>50% reduction in symptoms) and 9 in terms of remission of depression (score of ≤10 on the MADRSTooltip Montgomery–Åsberg Depression Rating Scale).
== Theoretical background == The acid dissociation constant for an acid is a direct consequence of the underlying thermodynamics of the dissociation reaction; the pKa value is directly proportional to the standard Gibbs free energy change for the reaction. The value of the pKa changes with temperature and can be understood qualitatively based on Le Chatelier's principle: when the reaction is endothermic, Ka increases and pKa decreases with increasing temperature; the opposite is true for exothermic reactions. The value of pKa also depends on molecular structure of the acid in many ways. For example, Linus Pauling proposed two rules: one for successive pKa of polyprotic acids (see Polyprotic acids below), and one to estimate the pKa of oxyacids based on the number of =O and −OH groups (see Factors that affect pKa values below). Other structural factors that influence the magnitude of the acid dissociation constant include inductive effects, mesomeric effects, and hydrogen bonding. Hammett type equations have frequently been applied to the estimation of pKa. The quantitative behaviour of acids and bases in solution can be understood only if their pKa values are known. In particular, the pH of a solution can be predicted when the analytical concentration and pKa values of all acids and bases are known; conversely, it is possible to calculate the equilibrium concentration of the acids and bases in solution when the pH is known. These calculations find application in many different areas of chemistry, biology, medicine, and geology.
== Recording == The recording of Dirt began in the spring of 1992. Producer Dave Jerden, who had previously worked with the band on their debut, Facelift, wanted to work with them again. He admired Layne Staley's lyrics and voice, and Jerry Cantrell's guitar riffs. The track "Would?" produced, engineered, and mixed by Rick Parashar, was recorded before the album, and first appeared on the soundtrack to the 1992 movie Singles. Dirt was recorded at Eldorado Recording Studios in Burbank, London Bridge Studio in Seattle, and One on One Studios in Los Angeles from April to July 1992. Dirt was recorded during the Los Angeles riots that erupted following the acquittal of four LAPD officers caught on camera beating unarmed black motorist Rodney King. The riots started on the first day of recording. The band was watching television when the verdict for the incident was announced. Jerry Cantrell was in a store buying some beer when a man came in and started looting the place. Cantrell also got stuck in traffic and saw people pulling each other out of their cars and beating them. The band tried to get out of Los Angeles without getting hurt while residents were protesting against police brutality. They took Slayer vocalist Tom Araya with them and went to the Joshua Tree desert for four or five days until things calmed down, then moved back into the studio and started recording the album. While recording the album, Staley, who had previously checked out of rehab, quickly went back to using heroin. Staley later went cold-turkey on his own while reading The Bad Place by Dean Koontz.
